About the Role:
We have an exciting fixed-term opportunity as an Outcomes Lead to drive the operation of a Supported Independent Living (SIL) home in Lilydale, Victoria. In this pivotal role within our Home and Living Team, you will mentor and guide a team of Disability Support Workers to deliver high-quality, person-centred outcomes to our residents. You will work closely with the individuals we support, their families/guardians and staff to ensure the highest level of care and positive outcomes.
You will be well supported by an experienced and committed Outcomes Manager and our Practice Quality and Safeguards team, who are experts in Zero Tolerance, human rights-based positive behaviour support, and a person-centred approach. Their guidance and coaching will empower you to lead with confidence.
This SIL home provides 24-hour active support to five individuals with varying support needs. You will lead a team of 13 permanent Disability Support Workers operating on a 4-week roster. Additionally, you will be supported by a Complex Health team to provide tailored support, including enteral feeding, epilepsy management, behavioural support, oral suction and other complex health care needs.
